How do I submit a medical bill?

Steps to Submit a Medical Bill:



Submit a Preventive Sharing Request (See your applicable preventive sharing details for eligibility)

  1. Determine Eligibility:
    • First, determine if the bill is eligible to be shared. Check your applicable preventive services, as shared services vary based on your membership and other plans. Some memberships may not include any preventive sharing, depending on your selection at signup.
    • For non-preventive services, will the cost of all associated bills related to this medical event exceed my Unshared Amount?
  2. Check Member Guidelines:
    • Ensure the bill is not excluded from sharing and does not fall under pre-existing condition limitations. Example: Excluded from Sharing are non-emergency mental health-related bills.
  3. Open a Sharing Request:
    • If the bill is eligible and it is a shared preventive service or exceeds your Unshared Amount (UA), you can open a Sharing Request.
    • This request involves filling out a simple form where you can upload your bills and notify ShareWELL about the medical services received or upcoming care.
    • This form can be found at ShareWELLhealth.org or in your member portal.
  4. Types of Sharing Requests: There are different processes with bills for care received versus upcoming care.
  1. Services already performed- Collect an itemized bill and request self-pay discounts. For emergency room visits, additionally request the medical records from the visit. Submit all documentation with your Sharing Request.
  2. Planned Healthcare- Many providers will ask for upfront payment for services that are being planned (maternity, surgery, imaging). We will work with you to prepay for them. Ask for a self-pay discount and provide good faith estimates to ShareWELL with your Sharing Request.
